The Bright Spots: Decoding the Success Model Behind Benue State’s Fully Restored Ambighir Primary Healthcare Centre.

    By DORIS DAVID ABIAA
    ‎
‎‎For several years, the narrative surrounding rural primary healthcare across Benue State followed a deplorable, depressing and predictable script shielded with decaying roofs, abandoned brickwork, non-functional boreholes and night time emergency deliveries conducted under the dim glow of phone flashlights.Across the vast agrarian expanses of the Middle Belt, where primary healthcare posts serve as the frontline defense against preventable maternal mortality, endemic malaria, and infant infections, structural decay was long viewed as an inevitable reality of rural living.

    Healthcare Centre

Without continuous refrigeration, the clinic could not store routine vaccines or vital thermal-sensitive oxytocic drugs needed to stop post-partum hemorrhaging. Routine childhood immunization schedules broke down completely. Mothers seeking immunizations for their infants were forced to either make expensive, day-long trips into Gboko town or wait for sporadic monthly mobile outreach visits that relied on melting ice packs.

To appreciate the scale of transformation at Ambighir, one must examine the decade of infrastructural breakdown that preceded it. Established to serve as the primary clinical hub for over 15 agrarian farming settlements scattered across Igyorov Ward, the facility gradually succumbed to deferred maintenance, administrative abandonment, and procurement failures

    Healthcare Centre

By early 2022, the facility’s physical infrastructure was severely compromised. Rotted wooden trusses caused large sections of the zinc roofing to cave in, allowing rainwater to flood consultation rooms, destroy paper health records, and submerge basic examination couches during every rainy season. The original hand-pump borehole had collapsed years earlier, forcing clinical staff and patient families to trek over two kilometers to fetch untreated water from local streams for wound dressing and sanitary maintenance.
At the Ambighir Primary Healthcare Centre (PHC)—a vital medical point along the Gboko–Buruku road serving agrarian communities across Igyorov Ward in Gboko LGA—that pattern of neglect has finally been broken.

‎Today, Ambighir Primary Health Centre (PHC) operates round the clock, powered by a stand alone solar energy grid, anchored by a high-yield motorized borehole and protected by a secured perimeter fence. Across selected project sites in Gboko and neighboring Buruku LGAs of Benue State, a quiet operational revolution is demonstrating how transparent procurement, community oversight and milestone based financing can transform a public capital allocation into functional, life-saving infrastructure.

We used to ask families to bring extra flashlights if a mother went into labor at night,” recalls Mrs. Comfort Terwase, a Senior Nurse and resident midwife at Ambighir PHC. “Managing severe postpartum bleeding or stitching tears in the dark while holding a phone in your mouth was a nightmare. Refrigerating vaccines was an everyday battle too. Now, our solar-powered cold chain operates non-stop, running water is available in the ward, and we can manage late-night admissions under full lighting.
Local residents share similar relief. Chief James Ordue, a community leader in Igyorov Ward, noted that security improvements restored public trust: “Before this project, the facility was pitch black and unfenced. Women avoided coming here after sunset. Now, the fence and floodlights mean staff are safe, and our families can access emergency care at 2:00 AM without fear.”
Local farmers, market traders, and community leadership have expressed deep satisfaction with the intervention, noting that the combination of perimeter security, clean running water, and reliable solar lighting has restored dignity to rural public health delivery in Gboko LGA.
The availability of a continuous cold chain means that routine immunizations against polio, measles, tuberculosis, and yellow fever can now be administered on demand, eliminating the past requirement for mothers to gather on specific days when ice packs were available. Furthermore, the capacity to manage complicated labor under proper lighting with immediate access to sterile water and emergency medications has led to a sharp decrease in home deliveries facilitated by untrained birth attendants
Situated along the busy Gboko-Buruku road, just before Ambighir village, Benue State, the vital healthcare facility which serves as the primary medical point of contact for surrounding the communities has been upgraded into a modern, fully functional health centre

    Healthcare Centre

The facility has no resident doctor; a visiting medical officer from the General Hospital at Buruku visits only once every month for complex case reviews. The facility relies on just two permanent CHEWs and one junior CHEW to manage outpatient triage, administrative intake, and preventive consultations. The facility relies on just two permanent CHEWs and one junior CHEW to manage outpatient triage, administrative intake, and preventive consultations.
In an official statement evaluating the project, the Executive Secretary of the Benue State Primary Health Care Development Board (BSPHCDB), Dr. Joseph Bem, underscored the institutional strategy driving these initiatives:
“What we have achieved at Ambighir Primary Health Centre in Igyorov Ward represents the baseline standard for primary healthcare provision under our current state health sector reform roadmap. For too long, public funds allocated to primary health centers were lost to fragmented monitoring and poor contractor compliance By locking disbursements to verified physical milestones, we ensure that every naira allocated yields tangible health infrastructure.”

The operationalization of Ambighir PHC has produced immediate, quantifiable changes in healthcare utilization metrics across Igyorov Ward and neighboring agrarian communities.
With the facility fully rehabilitated, health officials at the Ambighir PHC are preparing to expand maternal, pediatric, and general preventive services across Igyorov Ward and it’s neighboring areas. Antenatal registrations at the clinic have already risen by an estimated 65% within the first two months of full operational readiness, while infant immunization coverage across the ward is projected to reach historic highs.
The availability of a continuous cold chain means that routine immunizations against polio, measles, tuberculosis, and yellow fever can now be administered on demand, eliminating the past requirement for mothers to gather only on specific days when ice packs were available. Furthermore, the capacity to manage complicated labor under proper lighting with immediate access to sterile water and emergency medications has led to a sharp decrease in risky home deliveries facilitated by untrained birth attendants.
The contract was awarded to Apex-Line Integrated Infrastructure Ltd. following an open, competitive tender overseen by the Benue State Public Procurement Council.
The restoration of Ambighir PHC was financed through the state’s revitalized Special Intervention Capital Allocation for Rural Health Infrastructure (Budget Cycle Fiscal Year 2024–2025) under Budget Line Item
Public procurement records released by the Benue State Public Procurement Council indicate that following an open competitive tender, the contract for the comprehensive rehabilitation of the Ambighir facility Ref: BSSPPC/2024/BSPHCDB/REC-018) was awarded to M/S Apex-Line Integrated Infrastructure Ltd. at a total contract sum of ₦54,200,000.00.
To prevent contractor abandonment and ensure total compliance with engineering specifications, the Benue State Primary Health Care Development Board (BSPHCDB) replaced traditional upfront advance mobilizations with a strict Milestone-Based Tranche Disbursement Mechanism.

    Healthcare Centre
    MILESTONE-BASED DISBURSEMENT SCHEDULE (CONTRACT ₦54.2M)
Tranche 1 25% (₦13.55M) | Complete structural masonry repairs, roofing removal and replacement| with 0.55mm aluminium roofing sheets, and site clearance.
Tranche 2 35% (₦18.97M) | Successful drilling of high-yield motorized borehole to 80m, installation of 5,000L elevated tanks, and internal clinical piping.|
Tranche 3 30% (₦16.26M) | Full installation and load-testing of 10.0kVA hybrid solar grid, SDD cold chain refrigeration, perimeter fencing and floodlights.
    Final Retainage| 10% (₦5.42M) | Executive handover following 90-day defect liability assessment and joint civil society/community sign-off.
The transformation of Ambighir PHC offers a clear template for addressing Nigeria’s broader primary healthcare crisis. For decades, public health sector interventions suffered not from a lack of capital allocations, but from weak procurement enforcement, advance disbursement abuses, and a lack of community involvement during project execution.
By pairing strict public procurement transparency with milestone-tied financial releases and community-level oversight, the Benue State Primary Health Care Development Board has demonstrated that public funds can yield durable, life-saving infrastructure.
Unlike conventional capital projects where upfront advance mobilizations are disbursed without strict verification, the Ambighir restoration utilized a Milestone-Based Tranche Disbursement Mechanism
As Benue State advances its rural health reform roadmap, the transformation of Ambighir Primary Healthcare Centre stands out as a clear bright spot. It illustrates that when political will, transparent procurement, and community needs intersect, rural healthcare facilities can transition from symbols of systemic failure into beacon institutions that safeguard human life.
Local farmers, market traders, and community leadership have expressed deep satisfaction with the intervention, noting that the combination of perimeter security, clean running water, and reliable solar lighting has restored dignity to rural public health delivery in Gboko LGA.

Reacting to the development, community members expressed appreciation for the project, noting that the perimeter security and reliable power supply will allow the health center to operate effectively day and night.
